Agency Change of Details Form

In order to provide you with a quality service and to help us maintain accurate and up-to-date agent records, it is important that you notify us of any changes that may affect your agency facility with us.

Therefore, please ensure you notify us of any changes to your business, including: address changes and changes in company directors or partners. You can do this by completing the Agency Change of Details Form.

Once the change of details form has been completed, please return the form to: David Gibbs, Allianz Insurance plc, Distribution Support Team, PO Box 843, Halifax HX1 9HR.

Logo Licence form

It is now easier than ever to get a logo licence to use the Allianz name and logo. Simply submit your details in the form below and we will create a logo licence and post this to you.

 

Information

Once you receive the logo license in the post please take the time to read through the contract, sign and date it, before returning it to us. Once we receive the completed licence, we will email you the requested logos and you can then use these in accordance to the terms and conditions.
